What exactly are the terms of your written warranty? A 5-50 warranty ought to at least cover the engine and drivetrain fully for 5 years or 50,000 miles and your big headache appears to be the engine.
Are you still under 50,000 miles?
Your post seems to have information holes.
We need clarification to offer any meaningful advice.
The written terms of your in-effect warranty, be it the factory original or extended plan, mean everything right now.
The odometer jumping sounds off the wall. Are they claiming maybe it was tampered with, thus voiding the warranty?
If you have a legit warranty claim not being honored by any dealer, you need to go further up the chain at Toyota corporate.
