Please check your records for Account #433553948, for the addressee at 13204 Gurnee Avenue, New Port Richey, Florida. Account unpaid and payment due notices are continually sent to me at this address and forwarded to my current address. This bill was paid for the Bottom Line Year Book 2012 to the amount of $37.90 online via my Wells Fargo credit card account on 12-21-2011.

Please change your addressing records to my new residence: 6711 Embassy Boulevard, Apartment #324W, Port Richey, Florida 34668. This change of address may have caused the mishap in your records and caused the rebilling.
