Do not accept this offer, you will not receive your rebate. No matter what documentation you provide Cellular Choices rebate provider will deny your rebate and give vague / dishonest reasons for the denial.

With all the people being defrauded in all the different locations this will probably have to be settled by a class action suit.

The cell phone providers have an obligation, once notified of this scheme, not to activate or accept business from these liars, since the offers include a requirement to activate and maintain wireless service through one of the providers.
