I also purchased a Satellite A665D-S5172 from Amazon on July 3rd, 2011. Do you have the same model? It sat around until October when it was put into use. During the times I was using it I damaged the front left USB port. Is this the port they stated was damaged on your laptop?
In December it stopped working. It failed to power up. After diagnosis, I determined it was motherboard or processor and arranged to ship it to Toshiba through the tech support. In December it was sent to Toshiba for repair to the same address you mentioned in Milipitas, CA.
Someone named Gabriel Belmonte emailed me this:

This laptop was never touched by any water. I am on a deployment in the desert of all places and I certainly didn't spill water on it. It was sitting on my bed.

I have a feeling this repair facility is a third-rate shop that just disassembles a bunch of laptops all together and doesn't keep track of which parts go with which laptop.
