First Usa Bank applied for credit card $92.95 later no card and they say they don't know what i am talking about ; they say they have no record of this but on credit report they made inquiry and they deny it Westerville Ohio

i applied for a credit card over the phone ; they took $3.95 from account for processing fee ; 5 days later another $89.00 was taken from my account and one month later i still have no card ; i got credit report to find out who the company was because on checking account withdrawal it has no info ; and so on my credit report within the same time period First USA Bank had made inquiry on my account . i called First USA Bank and one tells me she shows where it was applied for but for some reason transfers me to another person who says she sees nothing then she transfer me again and nothing and finally the last lady said First USA Bank is an old bank and they don't have their records i asked why does it show First USA Bank made inquiry on my credit report and she said i can't answer that ; so now i am out money and wondering if there is a credit card somewhere it shouldn't be
