As a follow up to my previous telephone calls, please accept this as your formal notification to close the above referenced account effective immediately. Although my records indicate the outanding balance on this account is $3.95, I have enclosed a check in the amount of $6.00 which should cover any incidental fees.

Please send me written confirmation that this account has a zero balance and the date the account was closed. Additionally, include proof that you've complied with section 623(a)(4) of the Fair Credit Reporting Act by reporting this account as "closed by consumer" to the national credit bureaus.
