HB LOGISTICS MCGRIFF TRANSPORT INSURANCE DEPT CANCELED INSURANCE WITHOUT TELLING EMPLOYEES CULLMAN, Alabama

My husband worked for McGriff Transport/ HB Logistics for two in a half years. He had vision coverage with Superior Vision. His coverage was cancelled unbeknownst to him by HB Logistics. We had eye exams on May 2011. The eye exam was paid for on May 16, 2011. The prescription was filled  and filed on May 18, 2011. The insurance company refused to pay for the prescription. They stated that HB Logistics cancelled the policy on April 2011. We paid the insurance fee for two in a half years. Initially the insurance company was saying there was a misstake with the spelling of our names, for the provider to change the spelling and resubmit it. They still refused the pay the claim stating we were not covered during the time of service. This was done three times. I spoke with five different people at Superior Vison on this matter and three at HB Logistics. Their is noone working at HB Logistics currently that has been there more then 6 months. Noone knows what exact date, ( at least they aren't telling me) nor what month the policy was cancelled. I am still currently having this problem. The provider is threatening to send this bill to collections if the bill isn't paid. This has been going on for 6 months now and noone at HB Logistics will  help. None of the current employees knows what they are doing. This is out right ridiculious. What kind of business are they running. Mickey Mouse Truck Company.
