Asurion Cell Phone Insurance Asurion Charged $99 never sent phone, say they never received payment Nationwide

After paying the insurance on my phone throughout my contract with Verizon.  I dropped the phone and shattered the screen.  I went online and filed a claim on 4/30 put in my credit card information and was charged $99 for replacement phone.  The phone was supposed to arrive within 1 business day.  A week later I never received the phone and I called them.  They stated they never received the $99 and I need to pay that.  I told them that I had already been charged that.  

Two weeks went by and I called again, this time speaking with an operator and then supposedly a "supervisor"  they still claim that the did not recieve my payment and that I was more than welcome to pay the $99 to receive a replacement phone.  The supervisor stated that I would send my bank statement they would look at it, but all their records show that no payment was received, and there was no guarantee they would give me the money back because they never received it.  

I now have the Fraud Department with my bank disputing the claim.  So I am out $99 have a broke phone, and have paid for the insurance for the last two years of my contract.  Rip Off!!!!!
