DO NOT SIGN AND RETURN!!

This is Mail Fraud along with other fraudulant acts ... I was instructed to sign for a requested benefit; to receive up to $15,000. regards to Social Security. Not only does it ask for my signature, but they asked for my phone number, my age and if I had a spouse.

This is a dangerous document as it specifies a financial class of citizens. If there are any class acts cases, consider me in.
