Received letter from Countrywide Home Loan on 9/9/08 that former employee had sold my personal information to a third party: my name, address SS# and bank account #. I have notified my bank and the Triple Credit Bureaus on 9/9/08.

I was also informed by Countrywide to enroll in Triple Advantage and was supplied a Borrower Activation Code to fill out the online form to complete enrollment.

Please supply this form.
