I have a 2009 Camry, always do its maintenance in Toyota dealer every 5000 miles. Its current mileage is 23,000 miles.
A few days ago, my daughter drove it to local post office (3 miles away) and came back with a bare rim in left front wheel (driver side). Its tire fell off totally from the wheel. She was so shocked and felt panic for the day. Luckily that is within local street where traffic is not heavy and speed is not high. Should this happen while anyone driving it on the highway, he or she might lose their life.
I have had a few times tire punctured incidents in the past, but I never have any experiences to see a tire totally falling off from the wheel. Neither had I heard anyone that I know had this kind of horrible story. Maybe in movies, but never in real life.
Toyota Motor has been through quality downgrade for the past few years. Numerous quality complaints and recalls make me wonder what is going on with the company. I am not sure if I shall keep my faith in it.

That because the tire actually came off it is some manufacturer defect caused by Toyota. I guess it never crossed your mind that it could be something else.
The biggest is that by your own accounts those tires have been "touched" 4 other times, and the last time over 3K miles ago. Is it at least possible that something someone else did caused this?
Then you are getting the "story" from your daughter. Where based on your "she came BACK with a bare rim" seems to indicate that she drove between 3-6 miles on the rim. If not how exactly did she get back?

That's how far she drove with a bare rim As I said, I can never imagine the tire could complete fall off from the car.
This is very scary.

SO: The tire went flat, How is that Toyota's fault, Please explain.
The car was driven on a flat tire. How is it Toyota's fault that the driver didn't know she had a flat? Please explain.
The FLat tire fell off the rim, this will happen after a mile or two, How is that Toyota's fault, Please explain

You are actually coming to a public web site with your claims that somehow Toyota is responsible for this. Even if the tire just magically flew off your daughter still drove it at least 1/2 mile on the rim ONLY. As others have said most likely what happened is that the tire became flat and your daughter didn't notice it and because of that it eventually came off.
What is actually scary is your daughters lack of knowledge on how to SAFELY drive a car. Who ever taught her how to drive a car should have their license revoked.
I wonder what else your daughter isn't telling you.

running a tire flat until it comes off the rim....do you?
That or running it VERY low on air pressure and it popped off on a turn is about what had to have happened. The tires rely on air pressure to maintain a seal and contact with the rim (wheel). Without this, they come off and if driven further will disintegrate and come off the rim.
Do the people in Highlands Ranch know where you live so they can burn a cross in your yard?
