After filling out the 12 page form and sending it in, they insisted they never got it. I had to redo the originals, that entails getting my Dr. to fill out a lengthy form. Sent is again and they are again asking I have the Dr. fill out the same form AGAIN.
You cannot get anywhere on the phone with this company. It is a TOTAL waste of money and time!
Adds stress to an already stressful situation. This was supposed to give me peace of mind....
