I signed no release form to Shantay, Susan Deyoung , Mr engle at mh/mr nor Chris Minch a social worker at lebanon va in lebanon pa. mh/mr contacted veterans social worker without consent and both the mh/mr and csg and ihelp came to veterqan and said rudly were dropping you we spoke with Chris Minch from Lebonon va and he said there are programs at the va to help find housing. i have been homeless for 5 years and the lebanon va keeps throwing bricks at me to keep me honeless ont once has there been a social worker from the va that came to me scince 2005 and showed me an apartment. Mental Health Intensive Management at the lebanon va brought bug infested furniture to my dwelling and condemed by apartment and i've been homeless scince.
