It sounds like you have pretty good contact information, but I will list some separate info, just in case yours does not work:

It is better to WRITE to them, and make them sign for it, because they can say anything over the phone, which leaves you with no evidence. You can address that correspondance to the company management personnel: Mrs. Jeri Hale
It is also important that you fill out an online form at The Federal Trade Commission website: www.ftc.gov and their phone is 1-877-382-4357 You might want to call for extra pointers.
Please tell them this corporation is operating without a license.
When you submit your refund correspondance give them a 7 work day time limit. When they go past that make sure to pursue this further through the Federal Trade Commission, and consider the possibility of a Small Claims Court lawsuit, if it is not too much of a financial setback. I think they will give you a refund.
In addition, CHANGE YOUR ACCOUNT NUMBER, so that they cannot make more unauthorized withdrawals.
