Columbia House BMG Music Service placed themself on my credit report. I never bought anything from them, nor have I lived at the address mentioned Terre Haute, Indiana

Plain and simple, you were a identity theft victim.
Columbia house was victimized, also. Make sure you place a fraud alert on your credit reports.
Last year, I received some packages that I did not order. In one month, 3 dvd club and one book club memberships, and a small kitchen appliance. I called the companies who sent me the items, and found out that the credit card numbers were not mine. I made sure those accounts were marked fraudulent, and closed.
About a year ago, I was on a buisness trip , and used my laptop at an an Airport , with a wifi network. I had no financial info on my laptop, but had word documents with my mailing address. I think someone was hacking and trying to get card numbers, etc. > Since all they could get from me was my address, they used that to try out stolen credit card numbers.
Sure, it was a hassle to straighten out. I filed a report with the FTC. I was willing to give the items to the local police, but they told me that I should keep the items, that they could not tell a crime was committed.
Make sure you follow up with Columbia house, to get it corrected.
