I received a letter stating I had won $1,400,000 and all I had to do is sign the document entitled "Document of Award Payment Data Release Division" and send $19.99 cash, check or m.o. payable to D.R.D. Main Office Payments Director. It was signed by Robert Richards/Prize Announcer.

Mailing envelope had following address: P.O. Box 9035, Jericho, NY 11753 but return envelope for payment had a different address: P.O. Box 9004, Westbury, NY 11590-9004
