I should send the form W-915 and $20 for document fee to NBPI FOR THE Distrubution Authority. This is using the US Post Office to send these things out.
NBPI PO Box 98835 Las Vegas Zip 89195-0418 and the Distrubution Authority is P.O Box 98835 zip 89193-8835 I have the proof of using the US Post Office to send these things.
I have the complete mail package and will sent it e-mail to the authorities if needed.
