PREMIUM ADVANTAGE took money after I cancelled FORT LEE New Jersey

I cancelled same day and they attempted
in Dec 2004 to take money out and it did
not clear
2 times in Jan 2005 attempted again and
finally got the money out.
I called every week when to expect the
package never came.
Now I called on 2/16/05 and the number
is not in service.
They not only took my $299.95 but it
cost me $25 x3 for non sufficent funds
before they finally got the money.
