Personal Voice online scam for voicemail box billed via third party on your phone bill via ESBI Clearwater Florida

They hid their services in a credit card application form. Apparently when you sign up for this card. The phone company is asked to charge you for a voice mail box with Personal Voice. The charge is listed under ESBI (Enhanced Services Billing Inc.). There is no verification that the person filling out the online form want's the voice mail box or is infact the actual person themselves filling out the online form. The website they gave me for the credit card is http://www.mytryceracard.com/ but there is nothing on this site about a credit card. I called and was told that the only way to prevent this in the future is to call the phone company and block the billing.

For Verizon customer try emailing verizon.csc@verizon.com include both the billing telephone number on the top of the bill and the account number. Ask for the entire account to be blocked (no 3rd party billing).
