You are entitiled to your records DONT allow the statue of limitations , run out and be leery of any lawyer who cant get the records, go out of town, they are in bed together.
report the lawyer to the bar, is he wants to take your money and do nothing.

We did hired a lawyer.  And had to pay for records again.  Still do not have all of the reports.  
"ONE" Complete day of Medical Administration records are STILL NOT INCLUDED.  Hired a document expert who also said records looked altered.  Data received 5-5-10 missing from records received 3-19-12 and June 2012.  Hospital went back & changed allergies all the way back to 2004, and falsified my mother's signature on one of the reports.  

And with regards to the red ink and red marker writings - the red marker writings do NOT SHOW up on pictures taken with my cell phone or on the copies obtained.

AS for not understanding medical terminology - I was a medical technologist for 24 years. (thanks for HIPPA correction. mind & fingers don't always go in sync.) The records the lawyer used to determine if a "suit" was there were these "altered" records from SRRMC and from dad's primary care physician.  The lawyers did not obtain the records from other sources!  Used the 2 source that {in my opinion} at fault.  

If someone is allergic to Penicillin and told the ER staff that he was (mom & I were also there) you don't give him a penicillin antibiotic.  Within a couple of minutes dad had a reaction!  Then the hospital never told us about it!  Plus it was given 2 other times that first day!  And once again we were never told.  Date is missing on the records for HOURS!!!  
HIPPA law : to obtain true, unreviewed, unaltered, complete copies.  
NO TRUTH AND HONESTY LEFT IN THIS WORLD.

Its HIPAA not HIPPA. And i dont understand how they violated it? Maybe I missed that part in your report.

Second, just so you know. They can write with pen or marker on you medical records as long as they dont scratch something completely out that you cant read it, and write over it.

Honestly what I suggest, it seems to me you are a little over you head. I would contact an attorney that has experience in medical law to help you find exactly what you are looking for so you dont have to spend all that money on copies where you may not understand all the medical language in  anyway. This may cost you more money, but sounds like it would be more efficient.

UPDATE : Family went to SRRMC 3-19-12 to REVIEW medical records.  {Figured we would see true, unaltered, & complete medical records}  Even tho I had exchanged emails & phone calls with med rec's depart - SRRMC wanted us to PAY AGAIN!  Went there in person (persons) to dispute.  3-19-12 @1430.  (1) : med. rec. depart. girl said - had to have an appointment!  Yet NEVER ONCE had I been told that; via emails/phone.  Asked for supervisor; she said the same.  I said then let's go to administration since we had NEVER BEEN told, & it takes @30-40 mins to get there & my husband had taken off work for this.  
(2) : At @1515 were told they could arrange a nurse to sit with us & we were ushered into a room.
(3) : At @1600 I went out into the hall to find out why the nurse wasn't there yet.  
(4) : She arrived at @1610. {at least 1 1/2 hrs since we 1st arrived.}   Said they were working on coping records.  I explained how mother & I did NOT WANT copies, that we wanted to REVIEW the records with the copies we rec'd 5-5-10.    She said she must have misunderstood.
(5) : Another nurse finally brought in 2 files.  The first was @ 2inches thick.  The second @ 3inches thick.  As I started to dig out the rec's rc'd 5-5-10, the 2 of them kept saying we'll copy these for you.  I asked if these were dad's original, unaltered & complete med rec.  Informed yes & we'll copy them for you free of charge.  Nurse went to gather up files & I said just one for now. I'll look thru this other until your done.  As mom & I were looking thru records found Penicillin listed as ALLERGY!  WE HAD TOLD NRS & DR IN ER & ICU THIS, but on the copies we received 5-5-10 they had been DELETED! Took a pic with cellphone. Pages also had RED INK & RED MARKER WRITINGS!  TOOK PIC. Got yelled at/ I yelled back follow the law. This is ILLEGAL to falsify med rec.!  {Imagine my surprise when the RED MARKER WRITINGS DON'T SHOW UP ON COPIES OR PIC !}  Faxed SRRMC end of 3-2012 to stop CYA-ing & get us complete/unaltered/true med.rec's!  Couple day's later letter from SRRMC lawyer ordering me to stay away from SRRMC.  This is what I KNOW: 1. ER  gave dad wrong med 4-7-10 @1030 / @3min later dad-anaphylactic shock.  2. Took to Xray @1225 for CT w/contrast - yet partial records show emesis and N/G .3. Were told admitting for gallstone/hypotension. 4. Same day @1630 SRRMC gave dad wrong antibiotic AGAIN!  DAD- SEVERE ANAPHYLACTIC SHOCK due to amnestic response.  Record "NOT THERE" for time frame 1630 to 1800 4-7-10;  but I believe they did a CARDIAC CONVERSION. 1800 flowsheet shows 80/22 B/P !   I AM SO FRUSTRATED!  I NEVER IN A MILLION YEARS THOUGHT SRRMC WOULD CONTINUE TO LIE AND NOT TAKE RESPONSIBILITY.  Boy am I dumb.  There's more.  I'll post later. 
 

Dallas Thank you for your reply.  I HAVE notified JACHO,  But they have also informed us that what they find out will not be shared.  
And I must not have made myself clear . . . we need :
 a COPY of the ORIGINAL RECORDS that are NOT ALTERED. 
Copies for medical records were PAID for $625.00 @5-5-10.  

>100 of these medical record pages were duplicates.
 50-100 of these PAID for medical record pages are Unreadable - very dark. . .  They were copied by SRRMC so that half the pages are off the sheet.  Results are missing in the columns - like someone whited out ? cut out?  Falsified, altered, doctored, etc.

COMPLETE DAY OF ICU FLOWSHEET DOCUMENT NOT INCLUDED.

MEDICATIONS (MAR's) - different squares shaded differently, missing numeral documentation - (order numbers, etc.)  Known drug given - drug still there but time given is taken out. . . .
 
Copy of Medical Records paid for and not received.  A COMPLETE AND UNALTERED COPY.

This organization oversees hospitals and other entities.  FYI - Medical Records are owned by the facility not the patient(s).  You are not entitled to the original records - just copies and said facility can charge you per page to make copies for a request.  Also, Adult Medical Records can be destroyed per State retention without your knowledge or consent.
