There is a form with my name listed along with large sums of prize money on what is call a "statement of tracked documentation". At the bottom of that form, I am asked to return the "document release form" with $12 to the company American Sweepstakes Publishers, Department of Prize Data Information in Kansas City, MO.
I am very concerned that other senior citizens may not be saavy enough to realize this has nothing to do with Reader's Digest Sweepstakes. It is obviously a blatant scam.
