I called them several times and spoke with several people and they have denied that such offer ever existed. Anyone want to start a class action lawsuit against T-Mobile for falsely advertising free refill card in hopes to selling their product? This is the second time in which I bought a Samsung Galaxy S2 that didn't have an activation booklet. T-Mobile has the worst customer service that I have encountered in my lifetime.
