Write a complaint letter with the above information, include how many requests you made asking them to stop calling.
Sign and date that letter and send to your state's attorney generals office. Also send copy of letter to "the company" including who else you're sending the letter to. Specifically, in this letter, tell them this is your last attempt asking them to stop their harassing calls. Tell them they are out of compliance laws with the FTC and you will also be notifying them as well. Find the address to FTC and put that in writing as well and send.
You've now reported them and included the last time you'll ask them to stop harassing you. Both of those offices will INVESTIGATE their practices and possibly be fined a very large amount of money!

It's possible they are exempt from the do not call list due to you having done business with them in the past.
