Travel check voucher" offering a "certificate for 2 round trip airline tickets to anywhere in the continental US from any major US international airport." And "this check voucher must be redeemed by March 5th, 2013." The "check voucher" was made with "Pay to the order of US Airlineson behalf of (my name) for $1,298.00 With a memo stating "Must be Redeemed for travel certificate" the authorized signature was signed be "Jane Cae" the check voucher was dated March 4, 2013
