they show that you are eligigable for a rebate if you buy acuvue contacts and that the form will come in the box with your purchase.

then when you do get it you have to have had an eye exam within 90 days you dont qualify. this is very misleading if your are buying for the rebate. As it never said anything about an eye exam.
