hello  as i posted on the last time i said my confession about being involved in the scam that was on a patient that was tortured at a hospital in aug of 2009 and reall agh internal medicine didnt get involved untill october of 2011 and i was involved when a medical staff member was showing me how to handle the chart that was made for the patient and i tryed to tell them that i didnt want involved because i was afraid of going to jail but they also showed me how they did a cover up chart to use so that when this patient would go to another clinic they would receive the cover up one and a lady named jessica duck whom worked at a medical record clinic she was a electronic medical record tech she had all the this patients records from 1986 to 2008  distroyed and out of reach  and replaced the records with a electronic records only from aug of 2009 and up i just didnt understand what this patient did to them to make this happen but i wanted out of it and what brings me on this website is that i was on the computer in the office and i  noticed that the lady diana and cindy have printed out a letter to the patient asking that patient to sign a form for release of the chart that they are useing. and i  realized that what they are doing is trying to get the us department of health to investigate by using the cover up chart so they will see the patient as the fake chart was set up for the patient becaus the bottom line here is that this female patient was infact abused and many people was involved and that means that if the gov seen the patients real chart from 1986 through 2008 they will see that this patient is disabeled and they will see what the patient is now going through does not add up to legal care so what they did is made the cover up electronic chart to make it look like they are just learning of the patients health i just want to say 2 things #1 i hope this patient gets justice and #2 i know this patient is in danger if they dont follow her real chart so all doctors can continue to treat her medical needs as we all know that medical records play a big rule in care provided by doctors. i hope that this patient does not sign that for unless it was meant for her to be noticed by her real chart.
