Corporate Records Service "Annual Minutes Form" - fake form, but mailed from outside my state Frankfort Kentucky

Received a letter from Corporate Records Services. The enclosed form was a "2013 Annual Minutes Form" to include the names of all shareholders, directors and officers of my small business.

Here are some tip-offs that it is a scam:

On the front of the envelope, it reads, "This is not a government document"

It was mailed from a zip code outside my state; if the letter had originated/been created from the return address, which was in Frankfort KY, my state capital, it would have been mailed from Frankfort.

The zip code on the mailing stamp was "17406," which is for Emigsville, PA, just outsdie of York, PA.

The return address on the envelope is actually for a "Sun Tan City" in Frankfort, KY. I looked it up on Google Maps.

The name of the capital city, Frankfort, is misspelled ("Frankfurt") in the address of the "make check payable to" area at the bottom of the form.

So, I am 100% sure this is a scam. It even says it isn't a government doc, even though the emclosed form is created to look like a government form.
