In order to access an account a username and password are required.  In cases such as this, there is often another person that has been able to get the username and password from the user.  Users should keep their username and password private and should use a strong password (i.e. a password that is difficult to guess with a combination of characters and numbers).  It is also advised that users stay up to date with their operating system, browser, and anti-virus software by installing the most recent versions available.
