August 18, 2012 an agent from HCCUA named Barbara called me on my mobile phone, after enquiry of low cost health insurance. She explained HCCUA's medical insurance plan, after which I told her to forward me documentation about the plan to my mailing address. She asked my bank account number for a ONCE-OFF processing fee of I think $25.00, and mentioned the plan that would work for me would be $39.95 monthly. BUT, the monthly charge will NOT be billed until I received the medical insurance package by US mail, AND then I need to complete the Health Questionnaire section, as well as the Membership Billing Confirmation Letter. I made it very clear (with my wife next to me as witness), that I will NOT allow any monthly deduction from my bank account until I have received the package, studied it to decide whether this will work for me and my family, then subsequently (should I wish to do so) complete the forms, and return those back to HCCUA. She agreed to my statement made, and asked me to verbally state for the record (as recording) to the ONCE-OFF processing fee that will be deducted from my bank account. Which I did. 

Meanwhile, I did receive the HCCUA package in the mail, however at that time I did not study the contents at all, and NEVER complete any forms, and NEVER returned any forms back to HCCUA. I then considered the case closed, and NOT to pursue membership with HCCUA any more.

Today, November 27, 2012 I noticed an bank overdraft alert sent to my email, and upon checking my online bank statement, I noticed pending amount of $39.95 from Visa HCCUA xxxxx5400. Further investigation revealed that HCCUA had deducted a further 3 (three) payments of $39.95 from my bank account, started August 21, 2012. I was furious, and very puzzled.
 I contacted the customer service line 866-227-5400, enquired about this amount and was told that indeed my wife is a registered member of HCCUA, and 4x the membership amount has been deducted already from my bank account. I was never aware of these deductions until today. And very surprised that my wife 'became' a member, as I never authorized this legally with the completion of the relevant forms of HCCUA.
I asked for immediate cancellation of this membership. I ended the call.
Later the same day, I called the customer service line of HCCUA again, wanted to speak to the supervisor. I was transferred to a certain AMANDA. I explained the issue of WHY my wife became a member of HCCUA, WHY HCCUA decided to bill my bank account of $39.95 4x without my permission on a returned HCCUA form, whereby I was told that according to the recorded conversation between me and agent Barbara on 08/18/2012 I gave HCCUA verbal permission to allow the membership deductions from my bank account.
I asked her if she can forward me the proof about this permission, she would NOT let me have it. She refused. 
I demanded a full refund of 4x 39.95 ($160.00) deducted from my bank account over a 4 month period, for fraudulently accessing my bank account without my permission on their official form, whereby she denied my request.
Note that my family and I did NOT use ANY of the so-called benefits of the so-called insurance plan during this period, in which HCCUA unilaterally assign membership to my wife, without her (or mine) approval to do that.

They are still performing their song and dance. They did not and do not explain things well at all. It is difficult to express how I fell without the use of invective.
