Sleep institute of Utah Avoiding phone calls and no medical records Salt Lake City Utah

 I was a patient at the Sleep Institute of Utah and I've been trying to get a hold of them to get a hold of my medical records. They won't answer their phone and I know I need another sleep study but I mainly wanted the records and the result of the sleep study to give to my dentist so he can make a mouthpiece I can wear at night instead of wearing the CPAP mask. By the laws of HIPAA I am entitled to get my records but have no way of getting them. I want to know how I go about getting them,
