 Avonda Cooper, MSW is falsifying Medical Records for professional gain. In July 2017 a one minute phone call, asking for general information led Cooper to insert Behavioral Assessment Notes in a patient's Medical Record. Stating 1) that she did a Drug and Alcohol screening on patient 2) alleging a "in person" visit. 3) Did a review of ALL of the patients Records - completely unrelated to the one minute inquiry 4) claimed to have "Educated" the patient. Again, this was a documented ONE minute phone call. Apparently this is the norm at Barnes Jewish Hospital. This is Malpractice.
