Dear Debbie Flores – CEO:

 

RE: COMPLAINT # 2015-72932 TO MEDICARE FOR POOR SERVICES AND THE VIOLATING OF A PATIENT’S RIGHTS

 

On November 29, 2014 I was admitted to the hospital for seizures and a possible stroke. I received terrible service and my rights were violated. I checked myself out of the hospital on November 30, 2014 in order to save my life. I have filed a complaint #2015-72932 with Medicare. My enrollment number is #(((redacted)))51 and master account number is #(((redacted)))30.

 

My complaints are:

 

1. The hospital staff denied my wife access to me.

2. The hospital staff forced me into an MRI chamber against my wishes when I was having seizures which put me into a coma.

3.  The hospital staff failed to assist me when I was naked and unable to get up in the restroom. In fact the staff called a large group together to watch my embarrassing situation.

 

The hospital bill totaled $22,140.30 for the two day I was there. I have received a $175.00 bill but I was told that was waived. I have suffered immense pain and suffering because of the hospital’s negligence.

 

I suspect the hospital retaliated against me because I filed a Medicare Complaint against Del Webb Hospital for killing my mother on January 31, 2003. Medicare found the hospital guilty for her death because of withholding medication. The hospital made the investigation last longer than the Statute of Limitations. Medicare’s punishment was that it put Del Webb Hospital on an unannounced schedule of a six month inspections frequency for five years.

 

 

I expect justice from Medicare on your wrong doings. I expect Medicare to continue to lower your level of payments because of the hospital’s history of poor service record. My hope is Medicare revokes your Medicare License and protects to public from the experiences I have had with Del Webb Hospital.

LIVANTA HAS REOPENED INVESTIGATION AGAINST DEL WEBB HOSPITAL

We filed a complaint with Senator John McCain concerning the poor investgation done by the Medicare hired investigating company LIVANTA. Sente McCain wrote LAVINTA asking for information. LIVANTA'S contract is up for renewal and has many complaints. LIVANTA is now doing its job to keep the federal contract.

 

We just received a call from LIVANTA stating they have reopened my hospital complaint. They have discovered the orginial investigation only addressed two of my six medical care complaints. NOW Del E. Webb Hospital is under investigation again for poor medical service provided me. LIVANTA has already determined that the hospital failed to provide needed service immediately as required by Medicare. This poor medical service by Del E. Webb Hospital will continue to lower the hospital's declining grade. If this continues Del E. Webb may lose all Medicare Funding.

 

The hopsital's debt collector is hammering us for a disputed $175.00 charge even after we have disputed it. That is a $1,000 fine per violation of federal debt collection law. 

 

The public needs to know about the problems at Del E. Webb Hospital and its contiuning failure to resolve it for nearly a year.

Medicare does not fully investigate complaints

Medicare just spoke to me and advised me they can nothing if their subcontractor investigator, LIVANTA, closed the complaint. 

Livanta advised me the hospital had no record of my breathing problems and my blood sugar was fine. That is why the failed to supply the need medical equipment and medicine. BUNK!

The public needs to be aware that Medicare Complaints are a waste of time. SAD!

Medicare Investigation Failure

ADDITION: The recent Medicare Investigation of my poor treatment at Banner Del Webb Hospital failed to reseach why the hospital denied my wife access to me in the hospital. The hospitaL has our Medical Power of Attorney on filed but failed to follow the law and abide by the legal document. The hospital acted a a jailer who tried to kill a prisoner...which was me.

 

Medicare finds no problem with this illegal and dangerous medical service. GOD HELP US ALL!

Medicare's Hired Contractor LIVANTA found no problems

I just received the 08/04/2015 letter from LIVANTA (Medicare's hired complaint investigator) stating: "we determined the care you received met professionally recohnozed standards of care."

 

The findings were flawed because it said:

1. I never asked for a C-PAP machine and oxyen was not needed. The truth was that I was unable to sleep because I never received the needed breathing machine that I use every night.

 

2. I never received insulin for my diabetes because the hospital states it was not needed. The truth was that when I checked myself out of the hopspital my blood sugar reading at home was over 300. 

 

3. The Medicare Investigation failed to research why I was put into a MRI machine against my wishes when I was having seizures. I shook tha MRI apart and had to me strapped into the machine which put me into a coma. This violated my patients rights to be able to refuse treatment.

 

4.  The Medicare Investigation failed to research why I was not assisted when I was naked having a seizure on the hospital room bathroom floor. The staff just watch and laughed and failed to provide proper medical assistance. After this incident I checked myself out of the hospital to protect myself. 

 

This is the new Medicare Quality Innovation Network "LIVANTA." The bogus report was signed by Steven H. Stein MD. This new system is a failure!
