This is a total rip off company.  They are not contracted with any insurance company at all.  They push via phone to make an appointment and they don't tell you they aren't contracted.  Once you get diagnosed they have you talk to a financial person before they do anything else...  My friend came in for the same surgery I had with the same diagnosis without insurance and he paid 12k per arm.  They billed my insurance company over 120k for the same procedures.  Now I am stuck with 50k in debt because they wouldn't give me prices before I decided to have surgery.
