I also received a credit offer for $6,500.00 and found this when I searched for a website for CCS or CRS. Total scam, did not activate my card. They list their address as:

Credit Service Division
61 W. Utah, Ste. 63
Las Vegas, NV 89102
800-717-1278
On the back of the letter of approval, they state that a $99 annual fee will be charged "around 2 months after membership activation".
