I have been driving trucks of varied shapes and sizes for over 18 years. I have at least 10 years verifiable OTR experience. To this day, I have never been in an accident and my MVR is squeekey clean!

In May, 2006 Crete Carrier Corporation, a.k.a. CCC, and I parted ways.

After several failed applications with high valued trucking firms, I descovered that the reason for the rejection was because CCC had reported to DAC/USIS that I had been in a "preventable left turn accident" that allegedly occured in April, 2006. (Two months after I received a one year safe driving award letter from CCC).

There has never been any such accident...none where I was present anyway! Yet when I called CCC's safety, they told me that that is the report in my file and that they where not going to change or remove it...From their files or from DAC/USIS.

I'm not sure what my options are but I definately am not going to just drop it or stand for this type of defamation.

I know first hand what Gary is going through. God Bless you and your family. I was in charge of taking pictures of damage done to company equipment. I done this for 5 plus years and i have seen driver get points deducted from there driver points for accidents they didn't have but was a result from previous drivers. I can't remember a truck number or drive name but i can tell some stories including my own that would indicate unfairness in the company. This is a company owned by a lawyer who has been appointed to office by a U.S President. Good luck in the case huh? I won't give up either brother.

I'm new to the trucking industry and I've heard horror stories about bad DAC reports when leaving a company. The only thing I can say based on what I've heard is that if you're planning on staying in the trucking industry, don't leave the company you're with until you already have another job locked down. That way, the new company is seeing your DAC report before the old company has a chance to damage it. Good luck getting this resolved.

On-line letter form report request from DAC/USIS (Consumer Report Request Form)

AUTHOR: Kokomojo - (U.S.A.)

POSTED: Tuesday, August 05, 2008

You can actually go to the website of USIS, and get a blank form (Consumer Report Request Form) that is printable, and even downloadable for storage on a device for future use (My Documents). You will experience a long delay if you request that this form gets sent through the mail. You can challenge any entry that is made , and give your side. You can even hire a lawyer to do this for you.
Aftwer you've printed the form, make sure that the form is accurately filled out. Send this form to USIS through certified mail with a return receipt (green).
In the meantime, we should always keep in mind that we have an advocate (voice), and this is through OOIDA.com. We need to begin here to search for any legal assistance or advice that we might find.
Your situation i very similar to mine I'm actually working on my case before I submit my case on this site. This might even be as a result from "down sizing" that you see all around you. This has caused companies to cut back on the high end waged employees such as you and me in favor of those drawing lesser wages.
Let's go find out as to if this is what's going on. Here is the address of USIS:

USIS
P.O Box 33181
Tulsa, OK

What about a police report

AUTHOR: Steven - (U.S.A.)

POSTED: Friday, October 26, 2007

Seems to me that if they put that in the system they would have a police or accident report on file to back it up. Maybe ask for a copy of this or get a report number so you can check yourself. Maybe they mis-filed the report under your name instead of where it should have gone.

Good luck
