Easy Weight Loss Tea Sent Items after I cancelled out and did not submit form Internet

I started filling out the form to get the Tea for my daughter. 14 day free trial offer. Before submitting I saw at the end where they would charge me 37.00 after 14 days. I quit out of the form and chose the don't send box. To my surprise I was charged and sent the tea. I was going to send it back and tried to get in touch with the people but you can't. I have disputed the charges on my account.
I am now being charged again a total of 54.95 for something I didn't even order.
How they got my accoount info without me submitting the form I don"t know but now I have to go and dispute another charge.
