On March 4, 2013, my doctor wrote a prescription for Levothyroxine Sodium 75 mcg capsules.  Express Scripts changed the prescription to Tirosint Caps 75mcg tablets without my permission and I ended up paying a $70 co-pay because Tirosint is a brand name and not covered fully by my plan.

One of the pharmacists from Express Scripts told me it was perfectly legal for them to change my prescription without my permission and the reason they did this is because Levothyroxine Sodium 75 mcg capsules is not issued in capsules, only tablets.  It appears that the word "tablets" is a critical word because it prompted the pharmacist to change it and issue another drug which (1) it has the word "tablet" in it and (2) Express Scripts will make a lot of money if they issue a non-formulary drug.

Today my doctor was informed about the whole case and she was appalled.  She said Express Scripts just wanted the money and never mind that they are cheating senior citizens by pulling a shenanigan like this.

This is common practice for this company. If they are filling for patient's in states that do not have specific restrictions for filling, they will substitute generics. However, I have known of a number of complaints from patient's that a physician wrote a prescription such as this and instead of calling the physician, the company allows the pharmacist to take it upon themselves to fill it differently. In this case, instead of dispensing the generic, the pharmacist changed it all because the doctor wrote "capsule" on the prescription. This changes things greatly, it could mean the difference in the patient getting a lower cost generic and sending them a higher copay brand name medication. I have also heard of a situation where a physician wrote a prescription for a generic medication with a brand name strength. Instead of calling the doctor for clarification, (generic not available in that strength or did they intend the brand name medication) they fill it for the brand name medication in the strength that was specified on the script. I have heard of cases where this has caused unnecessary delay in the patient getting their medication and then when they do get it, it's not what they were expecting. I recommend that if you don't have to, do NOT go through Express Scripts. Also, never provide them with your credit card information. Always send in your payment after you get your medications!
