EZ1Rate Update on company Phoenix? Arizona

With phone calls to the company and with my bank I found that I had agreed to receive 1. Long Distance Service and 2. a $50 gas card. There has been nothing received including how to access the Long Distance service. A 10-day free trial ran out in June and they billed me $19.95 for Long Distance and $19.95 for an acquisition fee. I was billed the same $39.90 in July and have been promised a return of $19.95 if I can fax evidence of double billing. I still have no long-distance bill or gas card. We'll see how this turns out.
