Filled out all forms.  Enclosed Passport and Health Insurance card. Pain in full.  FedEx in envelope.  Received and signed for by VVOLGAL on 2-28-2017.

 

Received my Passport and health card back on 3-10-2017.  NO Social Security card was sent to me.  A letter stating I did not prove my ID.!!!

I sent all they asked for and paid in full only to received NOTHING from them.
