He forged my signature on Application for Adjudication of Claim dated 11/7/2011 to Workers' Compensation Appeals Board filed 2/17/2012.
He also submitted fraudulent and false statements on above mentioned form contrary to the facts I wrote on his Client Information Sheet that he acknowledged receipt by email on 11/8/2011.
