My representative for IAB Benefits was Mr. Choice Harris. Mr. Harris visited me in my home in Milwaukee, Wisconsin. Mr. Harris explained their program to me. I agreed to participate on 12/29/03. I was told that there would be a monthly fee of
$109.95 each month for this program. I was told that I would receive a membership package within 10 business days which would include my membership card. I did not receive a membership package nor did I receive any return calls from Mr. Harris regarding the status of my membership or my membership package from IAB. My bank account was not drafted in the month of January for payment. So I assumed that my membership was not active with IAB.

In the month of February my account was drafted for the January and February membership payments. I called Mr. Harris numerous times and left repeated messages for him regarding my membership package. Mr. Harris returned my call and left a message that provided me with his cell phone number, but no information regarding my membership or who I should contact regarding my membership. I attempted to contact Mr. Harris at his cell phone number. Again, Mr. Harris did not answer my call nor return my phone calls. Finally, I attempted to contact Mr. Harris from a friends telephone number and Mr. Harris answered his phone. I was told that it was some kind of mix up at the Corporate offices and he was not aware that I had not received my membership package. I requested a refund of payments made towards membership thus far, since I do not have a card to utilize this membership. I was told that I would need to contact the IAB corporate office to request a refund. I requested a contact number for the IAB corporate office. Mr. Harris did not provide me with that information.
