 The facility is owned by provider services.it haven't given out w2.Even thought the other facilities have.which are owned by same company.I know of employees who have called several times.also ask for corporate number which they won't give you.We have even let them know that they word be fined.they don't care.now we are left to file with government so that we are not I trouble with irs.this facility has hr person who could care less.i know of at least 59 plus employees who have not received them.
