mcpn Metro community provider network molly the PA filled out goverment paper work with false info. i went back in to ask y and the MA was rude told me that they where not ging to fix it and that i was mad becuse i wanted her to put i cant work. i have ssi case on file she marked it no she never asked me anything about it. she marked if i can sit stand ect with out asking me. didnt do my pap she said she would not fill out my med nine until she got proof did it then sent it off. she worye false info on goverment papers . aurora Colorado

 I had to go in to go in at nine 8/31/15 the PA is Molly Bailey I need a pcp so I go in back room the MA Thames my info Molly comes in sits down I ask her about the form med 9 she says I will not sign untill I get proof for my musculoskeletal disorders I said ok .she calls in gabapenten I fill out paper for my record that's it I bring back some records thinking she would wait until she gets others well she filled it out says Notting is wrong I can use my hands don't need to apply for ssi. she never asked me about work activities but filled that part out and I have had surgery for sciatic pain and to decompression of nerve. I have arthritis in back knee pain. she didn't wait for the record. then she didn't fill out job hours. she never asked me y I needed to have it filled out .she didn't assess me be for filling it out. she lied as well on government papers let robbin the MA be rude to me then gave wrong info for complaints .
