 I was working for a start up company that landed their first usa contract. This company named Octavian security was based out of the U.K. there headquaters are now in scottsdale . Well in the early year of 2014 they hired us from the previous security firm(allied barton) . Well the main and only boss at the time octavian had was Raul. When raul took our info for the new employee packets he had us bring in our drivers lic, social security cards,birth certificates. And we filled out our w4 forms. Well a week later most of our coworkers info mysterysly got "lost" well everyone had to give all the info back to him 2-3 in some cases well in all i never got asked. Fast forward to april, we now have the oppurtunity to get insurance they offered. They gave us aetna and aflac i was waiting for this time cause at the time and still currently battling depression from my mothers death. Well i got my insurences tge first 3 months or so since the open enrollment i would receive a check stub in person well i saw i had insurence cause in those stubs they where taking 75$ a pay period in total of up to 150 a month for the insurance. Mind you we still had to wait till june 1st for it to take hold. Mid june i checked myself into a rehabilitaion center cause the depression drove me to drugs and alcohol well as i was in detox i got rushed to the ER due to high blood pressure and anxiety 3 seperate times in 5 days while in the er the clerk could not find me in the system for insurence well i called my supervisor and followed the chain of command and i got assured carloa our new main boss by now said he will take care of it . I gave him my info name social and id. Well i started receving creditors collection bills from that week and i asked my superiors the peoblem once again i was reassured they (carlos) would take care of it. Well needless to say carlos got fired around the time i received my w-2 tax info that january well this is where the forgery starts with my w4 on my tax return w2 it said "manny" palacios not manuel corral palacios i have the original copy of that days w4 and it clearly says manuel corral palacios. And also my social number was written in wrong (((REDACTED))) is what they entered my real socoal is (((REDACTED))) well thats when i realized that raul had entered my info illegally by those two wrong infos. In that year i have already racked up over 25k in hospital and dr visits. When i called aflac that week i was in rehab to file a claim they also couldnt find me in system i had gotten the temporary diaibility from aflac cause im a type 2 dieabetic so i had already planned ahead cauae i had bad days and also the depression aspec is what is killing me i only wanted help getting the right counceling for my addictions and the big one depression. I never got any help from either insurance for almost 2 yrs we never got the oppurtunity to opt out of that insurance till this past january even when i opted out by telling now our new main boss brandon i did not want that insurence again he told me to sign a paper that said i was opting out well mid feb the prices of copper caused the mine to lay off over 200 employeez well being contractors we got the cut also me included. Well it took brandon oved a month to give me my final paycheck GRANTED i got laid off and waiting a month to get my check again cause the firat check he sent me had octavians address for my home addreas and thw check waa not signed. Well after two weeks of hearing he was overnighting my check i had to go into office to pick it up. And atill to this day i have yet to receive my 200$ deposit i am owed for turning it in. And also im owed my pto time im owed wich it amounts to 80hrs. They originaly called pto pto now they are saying that its not pto its vacation time. And they sont buy back vacation time. Its a mess in that company I forgot to mention i noticed in my last check cause i was not given proper credentials to chexk my paystubs the whole time i was employeed thru paychex or adp i was tge only one out of 28 employees with that problem thats why i could never see their screw ups.
