dear sir, im claiming the united healthcare grievance and claims filing repot today. plus legal services and claims despute. i want to say that i want to reinstate the policy aarp/medicare complete plan 1 with (10) additional riders plus dental platinum, H3659,pbp#003,  payer id#87726, health plan#911-87726-04,  member id#974649165-00 i want full reinstste ment for claims.  some didputes about late charges and not receiving payment by mail.  i want to clear these maters up and i want the policy back and late charges reinstation them. while on-line, i filled out several riders in addition to the up-dated policy. i have to have all of these things today. also the coupon book is rather expensive about $200. asking for dental/platinum rider plus cosmetic surgery,nursing home care,and added health reform riders too. also, $0 co-pay too. i have sent these papers in the mail to your office united health care and claims office too. i have not received any word from you yet. i want the dispute solved today and reinstste my policy AARP/medicare complete plan 1 plus(10) riders plus dental platinum today. asking help from legal services too. thankyou, thankyou, happy holiday, iris c, cleveland, ohio,44128.

dear sir, im claiming the united healthcare deluxe-rider coupon book re issued today. please follow up on this issue and please issue the deluxe rider coupon book that includes dental, cosmetic surgery, lasix eye surgery,eye glasses and silver sneakers fittness centers card too. thanks, thanks,also, my new id#(((REDACTED))) group#(((REDACTED)))  thanks, happy holiday, thanks, iris campbell.

United Health United Healthcare care I enrolled in uh dual advantage insurance program In their 2022 quick start guide it plainly state $0 copay and $400 allowance toward eyewear every year. When i got the eue exam and ordered glasses the walmart vision dept told me the $400 was for frames and plain lenses the glasses and lensrs i need to see through cost around 400 dollars and they would pay 169 dollars this is a bait and switch scam Salt Lake City Ut

Multiplan/PHCS I went out of network with , Horizon, united healthcare and aetna in june 2015, and to my suprise my bills where being cut by Multiplan, . When I contacted them they said I was in there network and they sent me a copy of a contract I signed when I was a sole proprieter, but 10 years ago I became a LLC and needed new tax ID, new busness, Multiplan never sent me a new contract and one of there own people told me that and sent me new contract. In october I was told I did not have to wait 90 days to get out , but I should not have been in,I am looking to recover lost income from June to end of October, also in October one of there people called me about a claim and asked me if my TIN# was my old # that was not valid so I know they have old out dated files. I sent certified letter to legal dept in October ,over a month ago and have heard nothing and multiplan will not give me the number, it is in bedford Mass. I am also concerned about 2 certified letters that I did not recieve from Horizon stating my exact dates to be out of network that they say where sent, I feel that everthing went through multiplan and that they stopped those 2 letters from reaching me, they where dated june 30th I recieved them by email oct22 from horizon with no return reciept # or signature new york New York
