I simply responded to a survey and next thing I knew I received a billing notice in the mail. I don't need or want additional long distance calling plan as I already have it on my cell and home phone.

I fell victim to the same scam in the very same manner. I responded to a survey and was signed up to a phone service that I didn't need or want. When I called my provider (AT&T) I was told I had to contact the original biller as they could not remove the charges, the billings company had to submit a credit. I also asked if I could block further companies from doing the same thing and was told they could not block third party billers (what a joke). When I contacted the billing party (USBI - Voicelink International) I was asked all types of information that I didn't readily have (such as the EXACT date of the charges). I eventually got the customer representative to state that I would be issued a credit in the next 1 to 2 billing cycles, that was on October 27th, 2008. I still have not received a credit and there is no live representative answering the phone anymore only a recording that repeats over and over the statement "if you'd like to speak to a representative press 1 now" and when you press 1 it loops back to the same statement. The fact the AT&T claims they can't block future ocurrences only leads me to believe that the whole communications field is in bed together to take as much as they can from the consumer that has few options (we can't go without phones). I'm quitting AT&T and am going to try my cable provider who offers phone service as well. I suggest to anyone else having issues with their phone service to switch to whatever other option you have. If companies start losing enough money they will respond.
