I would like to start by stating my belief that the product in question is a true fire hazzard and should be recalled and sales discontinued immediately to protect consumers from harm.
I ordered a 12 volt 5amp power supply via the web site 12vadapters.com. The website also indicates that the product is warranteed and will be replaced or refunded if there is a problem.
After 3 months of limited use. Maybe 10 hours total. The unit failed. I plugged the power supply into a relatively new battery charger and the charger immediately started to smoke. I had used the power supply before on the device many times without issue. Luckily I was right there and unplugged the power supply immediately or my house could have burned down. I later retested the charger with another power supply and it powered up normally(no smoke) and I could scroll through some LED menus but the charger was obviously damaged by the faulty power supply and could no longer be used.
I contacted Sam Dib the owner of 12vadapters.com (also samdib99 for his ebay store) He initially said he would refund me and honor his warrantee. However after several e-mails and providing him with the Paypal transaction ID and paypal reciept of sale, he refused to pay. I report this just to give an example of this merchants poor and unethical business practices.
I believe dispite the possitive feedback reflected on his e-bay site, which is posted a day or two after sale, that the long term use of this product could be a real hazzard. I believe further investigation is needed to insure the safety of the public.
Please look into this matter. I hate to think what might happened if this power supply failed at night in a house with sleeping children.
I now need a new power supply and a new battery charger to replace the one damaged by the faulty product. Finacially this is very difficult for me at this time and Mr Dib is unwilling to reimburse any damages. Not even a refund for the power adapter as stated in writing in his warrantee.

I work for 12vAdapters.com, and handled this client's entire transaction.
This client contacted claiming he bought an item from us 3 months before, and that it overheated. After investigation we found out he was running a 3 amp adapter with a 5 amp device, ensuring that it will overheat. We explained this. The client because immediatly rude and abusive. We agreed to refund him just to be done with it, it was only 25 bucks. He demanded 50 more dollars for the hassle. We did not accept this. He said he will take the basic refund.
We asked for his order number, or any way to find his payment, which we could not find under his name. He gave us an invalid transaction number. We could not find payment. I asked him to email us the confimration, or any proof of payment, he did not.
This person is irrasional and rude. He put my personal info on this site, but left his anonymous. Pretty dodgy.
