Dish Network, LLC Signature forged, then duplicated on multiple lines in contract Palatine Illinois

I have a contract with dish network which has a signature on it that "appears" to be mine, but was not a document I personally signed nor was presented. The signature is duplicated, very obviously, as it appears on several lines within a document I was not presented. The original document I signed (by electronic pad from the installer) was confirming the installation. The terms I discussed with the selling agent the day before did not include a term commitment (as my duplicated/forged signature is indicating I agreed to), nor was the charges what I agreed to. My forged signature was copied and pasted to 3 lines of a document I had not been presented with. 

In our disagreement, I argued the conversation I had was recorded and the recording would clarify the details of our verbal agreement and the details thereof. I spoke with 2 agents, then with a Jean Murphy of the Presidents office. I was basically told by all parties that these recordings could not be retrieved for such purposes. 

I cancelled my service, which was when I was made aware that they had a contract that I allegedly signed agreeing to a 2 year contract, and was subject to early terminatin fees. Up to this time, I was only upset that the charges to me weren't in line with what I was assured of.

I have read excerpts and reports that predate this one by at least 3 years, why has this not been dealt with or publicized?
