As others have reported, this is a very deceptive scam.  You think you are receiving a "gift" for completing a survey.  In my case it was associated with Verizon.  

The gift I chose was a watch for $6.99.  I thought that was the end of it.  I have since been charged $99.98 twice!

Stay away.
