Green Tea Extreme From Millionaire Mission.biz I ordered the trial bottle on the internet, for shipping $ only, never got it for almost 4 weeks, had Provo Utah

I ordered a trial bottle of Green Tea Extreme for the cost of the shipping which was only a couple dollars....then....I never received it for about 4 to 5 weeks, so could not cancel because I had no address or phone number, by the time I got it, my bank account was charged $74.95 for the next bottle! I was shocked but the phone number was on the statement, I called immediately and demanded to be cancelled and I wanted a refund to my bank account! The man said I had yo go to the web address and print out a cancellation form (PDF) fill it out and send it in to cancel, the only thing is, I could not get to the web address! I could not find it! Soooo, Now I am going to have to go to my bank and put a stop to any more payments being automatically taken out! It NEVER said the cost after the trial and I cannot get the cancellation form and the man on the phone would NOT let me cancel on the phone! I will probably have to cancel my account and start a new one!
